Are you ready for sports?

  1. Make sure you have a recent physical for 7th and 8th Grade athletes. (Click here for a list of locations.)
  2. Take the Salem Keizer School District Physical Form (English/Spanish) with you to the appointment.
  3. Register on FamilyID.
  4. Bring a mask and your own water bottle to all practices.

More information on Cross Country, Football and Volleyball will be available 10/13.
